Many guys on PrEP are not getting regularly tested for STIs

Two recent studies have highlighted how many men taking PrEP are not getting themselves regularly screened for sexually transmitted infections (STIs).

Less than half (43%) of the gay and bi men taking PrEP (Pre-Exposure Prophylaxis) in Baltimore, MD, were fully screened for the most common STIs (syphilis, gonorrhea, and chlamydia) when they started the medication.

The US study followed 290 men in the US city over a course of 30 months.

Of those surveyed, 7.8% tested positive for syphilis when they started PrEP. Participants also tested positive for rectal (11.1%), throat (7.5%) and urogenital (5%) gonorrhea, and rectal (11.7%), urogenital (5%) and throat (1.5%) chlamydia.

At biannual check-ups, researchers found that STI screening was lower and positivity for syphilis and rectal gonorrhea/chlamydia was higher. They said that whether the men reported having anal or oral sex impacted whether they had tests for certain STIs. This led them to call for greater clarification on STI screening guidelines for PrEP users.

PrEP, when used correctly, can minimize the likelihood of someone acquiring HIV.

However, many health organizations recommend those taking it to undergo regular check-ups. If PrEP users engage in condomless sex, they could be at an increased risk of picking up other sexually transmitted infections.

The US has seen a significant increase in the rates of syphilis, chlamydia and gonorrhea over the past five years.

A separate study from Germany discovered that one in nine PrEP users have not been tested for HIV or further STIs whilst being on PrEP. The results were discussed at the recent 17th European AIDS Conference and reported by AIDSmap.

Besides not being tested for other STIs, those one-in-nine users had also not had kidney function tests. Because the medication can have side effects in some people, it’s recommended to have a kidney function test regularly if you’re taking PrEP (the drugs are not recommended for people with certain kidney conditions).

Of those who did go back for tests, around 15% had an HIV test less frequently than every three months, and 8.5% went longer than the recommended six months before getting tested for STIs.

More concerning still, one in 25 of PrEP users had not had an HIV test before commencing their PrEP course. Anyone thinking of starting PrEP should always ensure they’re not already HIV positive – as the medication can cause the virus to develop resistance and become harder to treat.

Germany recommends PrEP users are tested for HIV and kidney function every three months and for other STIs at least every six months.

In the US, those on PrEP are advised to have an HIV test every three months. The CDC has previously recommended those on PrEP should have kidney function tests every six months.

Dr Will Nutland, of the UK-based organization PrEPster, told Queerty, “Regular STI tests – and HIV tests if someone doesn’t have HIV – are recommended for everyone who is sexually active, regardless of whether they are using PrEP or not.

“International guidelines make strong recommendations about having HIV and kidney function tests before or shortly after starting PrEP. If someone already has HIV when they start using PrEP, they are, in effect, taking sub-optimal HIV treatment and could be passing on HIV to their sexual partners.

“A very small number of people develop kidney problems when they use PrEP. A simple kidney function test will spot any problems and determine if it’s safe to use, or continue using PrEP.”

Utah’s governor has instructed officials to draw up new rules banning the practice of conversion therapy on minors. The new rules are now subjected to a public consultation period, but if they’re adopted, they could come into effect from late January 2020.

If so, it would make Utah the 19th US state to outlaw the practice on minors.

Local lawmakers have attempted to introduce legislation banning conversion therapy in the past. The latest attempt stalled in the state legislature earlier this year.

Conversion therapy is the discredited practice of attempting to change someone’s sexual orientation through psychotherapeutic means. It doesn’t work and usually does far more harm than good.

On Tuesday evening, Governor Gary Herbert, a Republican, proposed a statewide ban on the practice. He has directed the Division of Occupational and Professional Licensing, Utah Department of Commerce, to file the new rule, according to CNN.

His new rules adopt the same wording as the legislation proposed earlier in the year by Republican Rep. Craig Hall.

In a statement posted to Facebook, Herbert said, “I have learned much through this process. The stories of youth who have endured these so-called therapies are heart-rending, and I’m grateful that we have found a way forward that will ban conversion therapy forever in our state.”

Although the Hall’s proposed legislation failed, it crucially had the backing of the Mormon church.

The Church of Latter-Day Saints holds great political sway in Utah. Although it teaches that homosexuality is a sin, and is opposed to same-sex marriage, it backed moves to ban licensed therapists from administering ‘conversion therapy’ to minors on the proviso that its ministers could still offer advice if asked questions about sexuality.

The Governor’s move was welcomed by Rep. Hall, who said, “I urge adoption of the proposed rule so we can end conversion therapy once and for all in this state.”

Troy Williams, Executive Director of Equality Utah, said, “We are profoundly grateful to Governor Herbert and the Psychological Licensing Board for the thoughtful and meticulous in which they have worked to protect LGBTQ+ youth from conversion therapy.

“We have no doubt the adoption of this rule will send a life-saving message to LGBTQ+ youth across our state.”

The wording of the new rule will be published Dec. 15. The 30-day public consultation will end Jan. 14, and the new rule will hopefully take effect Jan. 22, 2020.

The complicated lives of the Drewitt-Barlows has taken another twist. The wealthy gay dads rose to fame when they became the first same-sex couple in the UK to be legally recognized as joint parents 20 years ago.

The two men became dads to twins through the use of a surrogate in the US. Commercial surrogacy is not allowed in the UK. The men have since gone on to father three more kids. They relocated to a mansion in Tampa, Florida, in 2017.

The men, with a combined wealth of over $45million, and their kids have been featured on TV documentaries and have often commented on news shows on matters relating to surrogacy.

However, last week it was revealed that one of the dads, Barrie, 50, had decided to split with husband, Tony, 55, after falling head over heels for the ex-boyfriend of his 19-year-old daughter, Saffron. The two men had been together for 32 years.

Besides dating Saffron, the younger man, Scott Hutchison, 25, was Barrie’s personal assistant for seven years. He is bisexual and says he never actually had a sexual relationship with Saffron.

Barrie says that his relationship with husband Tony became more platonic around 2006 when Tony began undergoing gruelling treatment for cancer. He is currently in remission.

Speaking to the Daily Mail about his new relationship, Barrie Drewitt-Barlow says he is still living in the same house as his former partner and kids – along with Hutchinson. He says all involved are accepting of the change in circumstances.

“I don’t want there to be any secrets and I want to get any negativity out of the way before our babies arrive,” confided Barrie.

“Yes, it is unorthodox, but we are a loving family and our priority is to have a stable home for our kids and what is now an extended family.

“It works for us – and if people are shocked, well that’s their problem, not ours.”

He says that although he wasn’t planning to become a father again, Hutchinson is keen to enjoy parenthood. Having made part of their fortune from running their own surrogacy advisory service, Barrie thought: why not?

“Scott has always wanted to have a child of his own so I said if he wanted us to be parents together, I would support him in that and have a baby with him.”

They now plan to use their sperm to fertilize an egg each. They will take advantage of sex-selection procedures, “in the hope that we could try for daughters – princesses to spoil.”

Allowing same-sex couples to marry leads to a significant drop in the rates of suicide in gay and lesbian people. That’s the findings of a major survey carried out in Sweden and Denmark.

Both European countries were early adopters of same-sex marriage (Denmark legalized it in 1989), which means researchers were able to analyze a large data pool. The study tracked more than 28,000 people in same-sex unions for an average of 11 years.

They looked at the suicide rates among those entering same-sex marriages between 1989 and 2002, and then between 2003-2016. The suicide rate fell by 46% between the two samples.

It’s been noted in other studies that married people – of all sexualities – are less likely to die by suicide, and the suicide rate in both countries has fallen in recent years. In the same study, the suicide rate for those in opposite-sex marriages fell by 28% between – still significant but not as great as the figure for those in same-sex relationships.

In their conclusion, the researchers note: “Although suicide rates in the general populations of Denmark and Sweden have been decreasing in recent decades, the rate for those living in same-sex marriage was declining at a steeper pace, which has not been noted previously.”

Although some other factors may be at play (for example, more effective treatment for HIV has also led to a drop in suicide among gay men) they believe that legalizing same-sex marriage has helped to decrease prejudice towards gay and bisexual people.

“Being married is protective against suicide,” said Annette Erlangsen of the Danish Research Institute for Suicide Prevention and one of the study’s co-authors.

“Legalizing same-sex marriage and other supportive legislative measures – they might actually reduce stigma around sexual minorities,” she told Thomson Reuters Foundation.

This isn’t the first study to suggest a link between marriage and a drop in LGBTQ suicides. A Harvard University study in 2017 found that suicide attempts by gay, lesbian and bisexual US High School students dropped 14% in those states with legal same-sex marriage.

The Swedish and Danish study wasn’t all good news, though. People in same-sex marriages were still 2.3 times as likely to kill themselves as those in opposite-sex marriages – with gay men more likely than any other group. Tackling stigma and prejudice still has some way to go.
