HIV Rates Rise Among Black Gay Men On PrEP

Citing a study in the  Journal of Acquired Immune Deficiency Syndromes AIDSMap recently reported that “Awareness and use of HIV pre-exposure prophylaxis (PrEP) among black gay, bisexual and other men who have sex with men (MSM) in the United States increased significantly between 2014 and 2017. The study also showed that PrEP was reaching individuals with high levels of sexual risk, sexually transmitted infections (STIs) and substance use, the very individuals most in need of this highly effective form of HIV prevention.”

Howeverthe report goes on to say:

HIV prevalence was significantly higher among PrEP users compared to individuals who were not on PrEP. The investigators stress this does not show PrEP failure but is likely due to factors such as poor medication adherence and lack of engagement with healthcare providers.

It is imperative to recognize that our findings reflect challenges to maintain proper usage of PrEP rather than biological failure of PrEP to protect against HIV,” write the authors. “HIV infections that have occurred among PrEP users are the result of suboptimal levels of drug concentrations because of challenges in adhering to PrEP medical regimens.”

Black MSM account for the largest proportion of new HIV infections in the US. If current epidemiological trends continue, 60% of black MSM will be HIV positive by the age of 40. Tackling the HIV epidemic among black MSM is therefore an urgent public health priority.

“The impact of HIV in the lives of Black gay men is as much a story of moral failure as policy failure,” Charles Stephens of the Counter Narrative told Towleroad.  “Any discussion around how HIV impacts the lives of Black gay men, must center racial justice. In the public health conversation, we are far too often blamed for the health outcomes we experience, rather than the failures of health care institutions. This has everything to do with race. Any programmatic, clinical or policy agenda to address HIV among Black gay men  must center our human dignity, be informed by our culture, address systemic barriers rather than individual behaviors, and resist deficit based messaging and pathological narratives about our community.”

Stephens added that he would caution against blaming Black gay men for “lack of adherence” and not sufficiently engaging healthcare providers, as to why the study found higher HIV prevalence among PrEP users compared to those not on PrEP . “We must ask harder questions of healthcare providers, including what efforts have been taken to eliminate institutional barriers to care for Black gay men. And are the service providers offering PrEP culturally literate on working with Black gay men?”

Teen Takes Broadway Musicals to the Next Level — While Mom Drives: WATCH

Need something funny to share with the fam over this Christmas holiday? Colin O’Leary’s ‘Showtunes 2018‘ mash-up is sure to start a few conversations. Colin O’Leary is to ‘Carpool Karaoke’ as Randy Rainbow is to ‘PBS News Hour’. Both take a staid and predictable format and offer it the drama and pizzazz of a Broadway musical.

And the other star in O’Leary’s clip, the brilliant foil, is Colin’s mother Carol (and was that grandma?), who plays the ‘straight woman’ to his over-the-top theatrics and also manages to be the most accepting and wonderful mom ever.

Supporting Our LGBTQ Family At Christmastime

Christmas is a great opportunity to gather together and celebrate our families and loved ones. I love this holiday: the lights, carols, food, family, decorated trees, cookies, stockings hung on the mantel, the Nutcracker Suite, the Charlie Brown Christmas soundtrack — all of it.

A special part of each Christmas is when I decorate our tree and hang the ornaments, many of which hold special meaning either because of how old they are or because of who gave them to us. I have a beautiful ornament from HRC that says “Love is Love” to remind my family of our strongly-held belief in LGBTQ equality. This year my husband and I will celebrate Christmas with my mom, my brother and my new sister-in-law.

It hurts me to think that some of my LGBTQ friends don’t get to celebrate Christmas with their families of origin. I love that many of my friends and colleagues have “chosen families”: people who love, accept and affirm them even if others don’t. I watched a beloved family member suffer until they finally came out as transgender, and each day I’m so proud of them for living their truth and finding their center.

Research has found that approximately forty percent of transgender adults consider suicide, often impacted by rejection from family and friends. This means acceptance is so vital and can literally be the difference between life and death. This statistic haunted me for a long time after my family member came out. I’ve seen some of our more distant family members turn away from our trans loved one.

It’s important for me as an ally to support my family and community in big ways and small. I speak up even when it’s uncomfortable because it’s important that I push past that discomfort. Our LGBTQ loved ones have spoken up about who they are — sometimes at tremendous risk to themselves — for years. Allies, who do not face the same risks, need to be visible in our support.

I have gained so much from LGBTQ people: a much broader understanding of sexual orientation, gender identity and gender norms. A deeper awareness of the damage that can happen when people are forced to hide parts of themselves. I’ve been shown love, patience and taught a true appreciation of Mariah Carey. I’ve been called out when I needed to be, and I’ve become better at my job. LGBTQ people don’t owe straight cisgender people any of this. We allies have a huge responsibility in making space for conversations where our LGBTQ siblings feel safe to be themselves.

It’s Christmastime and my relationship with Christianity is, in a word, complicated. But my main takeaway from Christianity is that Jesus taught to “love thy neighbor.” There’s no way that excludes LGBTQ people. I believe this beyond a shadow of a doubt — not just during this holiday season, but all year long.
